A green Volkswagen Sharan left the A141 at Westry on Saturday January 23 at 11.45am.
Police want to speak to the woman in case she has any information about the crash.
Pc Katie Jackson said: "We would like to speak to anyone who witnessed this, but in particular a woman who was driving a small white car who stopped.
"She was white and directed officers to the occupants of the vehicle further up the road but did not leave any details."
